Re-enrollment opens Monday, April 21, 2025
The information below will assist you to prepare for the 2025-2026 re-enrollment process. If needed, please call to schedule those physical, immunization, and dental appointments as soon as possible in order to meet the August 2025 deadline for re-enrollment. DO NOT WAIT as this will delay your student’s enrollment and may prevent a student from beginning school in September. Take your time and review the information below to determine if you will need any updates. If no updates are needed for your student's 2025-2026 grade level, then you should be good to go - the 2025-2026 re-enrollment packet will be a breeze!
Please review the "Documents You Will Need to Submit Each Year" list below. Some of these documents are forms you will need to print out and complete. Select each of the links below and print out the documents you will need to upload into your re-enrollment packet. The first two documents are by grade level – only one should be completed based on your student’s grade effective September 2025 (not current grade). If you are unable to print out the necessary documents, please stop by the Enrollment Office, and we will provide you with hard copies of these documents.
Documents You Will Need to Submit Each Year
Student Emergency/Medical Information Form (K thru 8th grades only)
Emergency Contact/Parental Consent Form (PreK 3&4 only)
Court/Custody Orders (if applicable, submit each year even if no change)
Medication Administration in School
● All medication both prescribed and over-the-counter must have a completed medication administration form filled out by a licensed physician every school year and uploaded into the re-enrollment packet
● Medication must be in the original container labeled properly by the pharmacist. You can ask the pharmacist for a duplicate container if needed. Over the counter medication must be in its original package and unopened.

FROM THE SCHOOL NURSE:
Updated/current forms must be uploaded into FACTS re-enrollment packet for the following grades:
KINDERGARTEN --This includes students returning from PreK4
1. Immunizations
● DTap-4 doses (last after 4 th birthday)
● IPV-4 doses
● MMR-2 doses
● Hepatitis B-3 doses
● Varicella-2 doses
2. Physical Exam - Completed by child’s pediatrician
3. Dental Exam - Completed by private dentist
3rd GRADE
1. Dental Exam - Completed by private dentist
6th GRADE
1. Physical Exam - Completed by private pediatrician
7th GRADE
1. Immunizations
● Tdap (Tetanus, Diphtheria and Pertussis)
● MCV4 (Meningococcal)
Most students received these immunizations during 6th grade physical exam. Please prepare that document of immunization for upload. If you know that you did not upload an up-to-date immunization record during the 6th grade year (or maybe you handed this in in hard copy at a later date), please upload updated immunizations.
SCHEDULE the immunization update appointment as soon as you can!
2. Dental Exam - Completed by private dentist
ALL STUDENTS, regardless of age, should be up to date on their grade requirements
When Re-enrollment Opens and You Are Ready to Begin
Please go to your FACTS Family Portal and log in using your selected credentials. If you are asked for the District Code at login, this is SKS-PA. From the left side menu, select Apply/Enroll, then Enrollment/Reenrollment, then "click here to open enrollment", then select the student for whom you are completing the re-enrollment. Repeat this process for each student you will be re-enrolling.
